DIRECT SERVICES

Direct Services is the top funding priority for the Chanda Plan Foundation. Improving the quality of life for persons with physical disabilities, our mission. We provide acupuncture, massage and adaptive yoga services at the Foundation through our Location Based Services. Through a quality and cost controlled environment we are able to provide more services to more participants and make a bigger impact. Adaptive exercise and other services performed nationwide are offered through a network of providers through our Provider Based Services. In 2015 we provided 3,927 individual treatments, saving participants an estimated $208,496 in out-of-pocket costs.

Program applications are reviewed every 3 months.

Terms of service for program re-application, now state program applicants must wait 1 year after last treatment date before they are eligible to re-apply. There is also a lifetime award cap of $10,000 per participant.
